Complete the Create a Ticket<\/strong> form:<\/span><\/p>

Origin<\/em> - Where did this come from?<\/span><\/p>

Contact information - <\/em>Who is the customer, and does he or she want a response?<\/span><\/p>

If the person wants a response, enter name and email and\/or phone.<\/span><\/p>

If left blank, this Ticket is submitted anonymously.<\/span><\/p>

Note: If a name is entered, a phone number or email address is required.<\/span><\/p>

Received from<\/em> - What type of customer is this?<\/span><\/p>

Assign to - <\/em>What Topic (team) should respond?<\/span><\/p>

You may identify a specific user to receive this Ticket. If no user is specified, the topic owner will receive it.<\/span><\/p>

Type<\/em> - What type of Ticket is this?<\/span><\/p>

Content<\/em> - Subject (What is it about?) and Description (What is the actual question\/comment\/etc.?)<\/span><\/p>

Attachment - <\/em>Any relevant documents<\/span><\/p>
